When scheduling a video telehealth appointment, Please confirm your phone number and email address.

Do you have a scale, thermometer, and/or blood pressure monitor? If you do, please have your readings available for your video visit.

If you have an iphone, ipad, or Mac computer, we will be using Facetime to perform your video visit.
